2% COLA Questionnaire for FY 2004-05

The Pennsylvania legislature included a 2% cost-of-living-allowance (COLA) for mental health/mental retardation/drug and alcohol (MH/MR/D&A) programs in the commonwealth’s fiscal year (FY) 2004-05 budget. PCPA is interested in documenting how this provision of the
FY2004-05 budget has actually been implemented for existing MH/MR/D&A services.

Please help us document whether all eligible organizations have received the COLA by taking a few minutes to complete the attached survey. Your response will help PCPA develop its next steps in making a COLA a part of future state budgets.

Those organizations with membership in PCPA and another statewide association may receive this questionnaire twice or in a slightly different format. PCPA respectfully requests that each member receiving this questionnaire be sure to complete the PCPA version for the association. PCPA and other statewide associations working on this issue will bear the burden of removing any duplications when compiling the information for the overall service systems. However, having separate association data will assist PCPA in preparation of its own materials and in PCPA-specific documents.

Additionally, if multiple persons receive this questionnaire within an organization, please feel free to coordinate your responses to one form. Nevertheless, should PCPA receive multiple responses from a particular organization, the association will do follow up to ensure that one accurate response is received for each member organization.
