DPW Youth Suicide Prevention Survey

The Department of Public Welfare (DPW) suicide prevention advisory committee is requesting assistance in completing the Pennsylvania Youth Suicide Prevention Plan Survey, which is available from PCPA by contacting Connell O'Brien. The survey is designed to collect information and comments that will be used to update the DPW five-year action plan, assess progress made towards primary objectives, identify barriers, and prioritize future objectives. The information requested is based on activities implemented since the last plan update in March 2004.

Agencies and individuals completing the survey are asked to:

The Pennsylvania Youth Suicide Initiative is a multi-system, statewide project comprised of stakeholders representing state government agencies, private and public community organizations, provider agencies, advocates, and survivors who work together to direct, promote, and support suicide prevention efforts identified in the Pennsylvania Youth Suicide Prevention Plan. The comprehensive plan and the five-year action plan can be viewed or obtained at www.sap.state.pa.us.
