Advance Directive Booklets Now Available

The Advance Directive Booklet developed by the Disabilities Law Project for a coalition headed by the Mental Health Association in Pennsylvania and the Pennsylvania Mental Health Consumers' Association is now available as a PDF document (access the link below). It will be available in printed form within the next few weeks. This 56-page booklet includes answers to frequently asked questions, three forms (Combined Mental Health Declaration and Power of Attorney, Mental Health Declaration, and Mental Health Power of Attorney), instructions for each form, and a card on the front cover that can be removed and carried by the individual. In the printed version, the forms and the card will be perforated for ease of removal.

Frequently asked questions for providers are also being prepared. A document is available, but has not yet been formatted for printing. Instructions for agents will also be developed. More information will be provided as the documents are finalized. PCPA members may contact Betty Simmonds with questions.
