Centre County Mental Health/Mental Retardation has issued a Request for Proposal (RFP) for the provision of full care community residential rehabilitation services for two male consumers, with the possibility of services for a third consumer should additional funding be available in the future. An annualized amount of $160,000 was allocated for residential services for two consumers, which will be pro-rated for the remainder of the fiscal year. Proposals should include cost projections, staffing patterns, and a proposed program format that addresses the needs of the consumers and families involved.
Those interested should submit five copies of the RFP response to Carol Waltz, Administrator, Centre County MH/MR Office, Ground Floor Willowbank Building, 420 Holmes Street, Bellefonte, PA 16823. Responses must be received by 4:30 p.m. on December 13 in order to be considered.
