The Office of Mental Health and Substance Abuse Services (OMHSAS) will once again conduct the annual Consumer Satisfaction Survey required by the Community Mental Health Services Block Grant. The survey is developed by the Centers for Medicare and Medicaid Services (CMS). The survey instrument is distributed to a random sample of individuals who had received at least three behavioral health services between July 1, 2004 – June 30, 2005. Adults and family members of children and adolescents who received services are included in the sample. This year 14,400 surveys will be sent to adults and 7,400 to family members. Also, a subset of 2,000 randomly selected adults who have received behavioral health services will be asked to complete and return the Recovery Oriented System Indicator (ROSI) survey. The Consumer Satisfaction Survey will be distributed in late September or early October. Respondents are asked to complete and return the survey in the postage-paid, self-addressed envelope within three weeks of its distribution date.
Providers are asked to encourage individuals to complete and return the surveys if they have received them.
