COMMCARE Waiver Providers Needed
December 8, 2004

The COMMCARE waiver is a home and community based program developed for individuals with a diagnosis of traumatic brain injury (TBI). This waiver is open to Pennsylvania residents ages 21 and older that have a medically determinable diagnosis and are eligible for special rehabilitative facility level of care. The disability must result in substantial functional limitation in three or more of the following major life activities: mobility, behavior, communication, self-care, self-direction, independent living capacity, or cognitive capacity (judgment, memory, reasoning).

The Department of Public Welfare's Office of Social Programs (OSP) is looking for providers and agencies to serve individuals eligible under this waiver. Any provider interested in offering services through this waiver may contact one of the following individuals based on the region where they provide services:

  • Personal Care Services
  • Respite Care
  • Habilitation
  • Environmental Adaptations
  • Non-Medical Transportation
  • Assistive Technology and Specialized Medical Equipment
  • Therapies (Physical, Occupational, Speech, Part-time Nursing)
  • Structured Day Programs
  • Community Integration
  • Behavioral Specialist Consultant
  • Cognitive Therapy
  • Chore Services
