Stakeholder Planning Team Request for Applications
September 2, 2004

The Pennsylvania Department of Public Welfare organized the Home and Community Based Stakeholder Planning Team (SPT) in 2002. The SPT is now being restructured and redefined. It will move from being a planning body addressing a broad range of issues to a more focused group of diverse stakeholders that can address cross-disability issues. The focus will be on cross-disabilities issues and on helping to assure that people of all ages with complex cross-department, cross-office needs will be served.

Medicaid funded home and community based services in Pennsylvania are:

  • Infants, Toddlers, and Families Waiver (Early Intervention)
  • Consolidated Waiver for Individuals with Mental Retardation
  • Person/Family Directed Support Waiver
  • Attendant Care Waiver
  • Independence Waiver
  • OBRA Waiver
  • AIDS Waiver
  • Michael Dallas Waiver
  • Elwyn Waiver
  • Aging Waiver
  • Commcare Waiver
  • Long-Term Care Capitated Assistance Program
