The Pennsylvania Department of Human Services (DHS) has released updated communication for licensed providers who hold Certificates of Compliance from the following offices:
- Office of Children, Youth, and Families (OCYF)
- Office of Long-Term Living’s Bureau of Human Services Licensing (OLTL/BHSL)
- Office of Mental Health and Substance Abuse Services (OMHSAS)
The
Checklist for Initial Application for a Certificate of Compliance in a Human Services Setting has been updated to include the step-by-step process required to apply for a Certificate of Compliance/license.
A separate
Checklist for Name Change on a Certificate of Compliance in a Human Services Setting has also been released. When considering a change to the Certificate of Compliance, providers must adhere to the requirements of
55 Pa. Code § 20.57 and § 20.58. Upon review of the application for a Certificate of Compliance (HS 633), DHS will decide if the name change can be applied to the current license, if it requires a new license, or if the request will be denied.
Additionally, the
Verification of Affiliations form, as required on the checklist, is also available.

Please be advised that, pursuant to 55 Pa. Code § 20.57 (relating to restrictions on the Certificate of Compliance) and § 20.58, (relating to notification of change), a certificate of compliance is not transferable. DHS must be notified prior to making any changes that will affect the certificate of compliance, including but not limited to facility/agency name changes, legal entity changes, or change of ownership. Changes to the Certificate of Compliance must be approved by DHS prior to implementing any of the above organizational changes. Noncompliance with these provisions may result in revocation, nonrenewal, fines, or other enforcement action.
